1. Reason for consult (longevity, weight loss, peptides, exosomes)

2. Advocate name (and number) placing consult

STEP 2: Send the following information or PDFs containing the following to consult@eternaregeneration.com

• Diagnoses and past medical history

• The most recent set of executive panel and specialty labs

• Any recent body composition testing / VO2 max testing

• Any imaging pertinent to a focused complaints (ie joint pain)
